ELIGIBILITY FOR SERVICES
The Disability Resource Center is the designated office on the campus of the Dixie State College of Utah to evaluate disability documentation and to determine eligibility and implement reasonable accommodations for students enrolled at the College. In order for students with disabilities attending Dixie State College to be protected from discrimination and receive accommodations and support, they must qualify for services under the mandates of the Americans with Disabilities Act of 1990 (ADA). It is the sole responsibility of the student to provide documentation for the purpose of determining eligibility for services.
The student begins by reqesting accommodations from the DRC Coordinator. The Coordinator and the student will determine what accommodations will suit the students needs. The student will need to provide written documentation of a disability in the form of a report before most accommodations can be approved. The written report should be dated within three years, and typed on professional letterhead. It also should include:
- The name, title, credentials, and signature of the qualified professional writing the report.
- A history of the individual's disabling condition.
- A diagnosis of disability - Dixie State College policy requires documentation that reflects a battery of test results: they mirror the adult based norms (WAIS-III) that are required to determine eligibility for all post secondary education. This is primarily derived from the Diagnostic and Statistical Manual (DM-Version IV), or the International Classification of Disease (ICD), where appropriate.
- A description of the student's functional limitations as a result of the disability and/or treatment demonstrating it limits one or more major life activity, such as learning, walking, sitting, standing, seeing, hearing, speaking, breathing, reading, writing, working, performing mathematical calculations, or caring for oneself.
The diagnostician must be an impartial individual who is not a family member of the student, and must be a medical doctor, licensed clinical social worker, or a licensed psychologist.
