Disability Resource Center

EXAM ACCOMMODATIONS:

The Disability Resource Center provides an exam accommodations form in triplicate to students with an applicable documented disability. Students will then present this document intact to the professor who will then complete the instructor portion of the form i.e., list of allowed items to be used as aids in the exam process, etc. This completed document will then be returned intact to the DRC to provide the support.

POLICY:

Exams will be scheduled in accordance with the following DRC policies:

• Exam Hours: Monday - Friday: 8:00 am - 5:00 pm
• To schedule an exam (date/time) a scheduling form must be completed at least 24 hrs. prior to the requested date/time.
• Changes in exam accommodations must be authorized by contacting the Coordinator in the DRC office:  Call (435) 652-7880, or email drc@dixie.edu.

Students

Exam accommodations are available based on documentation of a diagnosed disability. Students qualifying for exam accommodations should meet with the DRC Coordinator to receive an exam accommodation form for each course for which they are seeking accommodations on a semester basis.

It is the student's responsibility to:

• Present the exam accommodation form to the instructor or professor.
• Complete the student portion of the form.
• Meet with instructors or professors to complete the instructor portion of the form.
• Return the form to DRC at least five school days prior to the first exam in order to guarantee the availability of all approved accommodations.
