IF you have been selected for verification for 2009-2010 school year:  Print, complete and sign the appropriate worksheet, and return the form with a signed copy of your, your parents, or spouses 2008 Federal Tax form 1040, 1040A, 1040EZ or Telefile.

• DEPENDENT  Student Verification Worksheet   If you were required to use your parent’s income information on your FAFSA you will need to complete the Dependent Verification Worksheet.
•  INDEPENDENT Student Verificiation Worksheet    If you were not required to use your parent’s income information on your FAFSA you will need to complete the Independent Verification Worksheet.
•  CONFORMATION of Independence Status   This form to request Independent Status
•  AUTHORIZATION  to Credit Account  This form Is required from all students receiving Financial Assistance at Dixie State College.  
• RE-EVALUATION Request Form To request changes to your financial aid award i.e Accept financial aid awards, Cancel financial aid and/or loan awards, or Request additional loan amounts from remaining eligibility of loan award.
• APPEAL for Special Circumstances   You or your family have special circumstances that should be taken into account for consideration of financial aid eligibility.
  
• LOAN COUNSELING  Student loan counseling is required for all borrowers at Dixie State College for each year loan is requested. 
• LENDER SELECTION Form You must select a lender before we can process your student loan.

• LOANPLUS  INFORMATION (LIF) If a parent is interested in applying for a parent PLUS loan, the parent is required to complete the Loan Information Form and return it by mail or fax to Dixie State College, Office of Financial Assistance. Fax (435) 656-4087

•  APPEAL for GPA-Earned Credits  Satisfactory Academic Progress (SAP) must be maintained to receive financial aid.

• CONSORTIUM AGREEMENT  To receive financial aid while attending two schools the same semester, complete and return this form for clearance to our office.

• FERPA (The Family Educational Rights and Privacy Act)  is a Federal law that protects the privacy of student education records.  Under FERPA, a student must give written permission to have another individual such as parents, guardian or spouse, etc. access their Dixie State College information. If you would like to waive your FERPA rights by allowing a third party to discuss and/or have access to your education records, written permission is required.   The FERPA Release Form is available at http://www.dixie.edu/reg/info-release.doc.    Please submit the completed form directly  to the Registrar’s Office.
  
• DEGREE AUDIT REQUEST - Total Credits
• WUE   Western Undergraduate Exchange contractual agreement required for students who have been awarded a WUE scholarship through the admissions application process.
