Educational Plan Worksheet

The Associate Degree Nursing program prepares students to assume the role of registered nurse. Graduates of this program will receive an Associate of Applied Science degree and are eligible for registered nurse licensure upon successful completion of the NCLEX-RN® licensure examination.

Students who successfully complete the prerequisite courses are eligible to apply for the ADN program.  Once you have completed your prerequisite courses, the ADN program may be completed in three semesters.   Students will receive an Associate of Applied Science degree in nursing (ADN). Students admitted into the ADN program may continue through the BSN program as long as they remain in good standing. There may be additional co-requisite or general education courses required for the BSN program.

Because of the length of the ADN nursing program,, we are not able to grant advanced placement for licensed practical nurses. Transfer LPNs will be considered with the general applicant pool of students for the ADN program.

Applicants will be admitted into the ADN program without discrimination as to age, sex, creed, ethnic origin, or marital status.

Local students should plan to attend one of the information meetings prior to making application for the nursing program.  The meetings are held in the Taylor Health Science Building - first floor auditorium at 3 pm. 

Dates are as follows:

     March 30th, 2012

 

Applications will be accepted during the following dates:

Semester Admission Dates
Fall February 1st - March 31st
Spring July 15th  - September 15th

Applicants will be notified by email  (sent to dmail account only) of their status between May 5th and May 31st for fall semester, and September 20th and October 31st for spring semester. No other notification will be made.

Space in the program is limited, therefore, admission will be competitive and applicants will be scored on an objective point procedure that has been strategically set up to predict student success.

Academic Excellence
A crucial element for success in a rigorous ADN program is academic excellence in prerequisite courses. Students must complete all prerequisite courses with a minimum “C” grade, and have a cumulative GPA of 3.0 in those courses.


Prerequisite Courses

Prerequisite courses for the ADN program provide students a solid foundation of knowledge and are essential for success in the ADN program. Program eligibility will be determined based on GPA and completion of the prerequisite courses. It is recommended that transfer students consult with the nursing programs advisor  for more information.

Classes may be repeated only once. A student who has repeated any prerequisite course more than once, and neither grade has met the minimum requirement of "C" or higher, is ineligible to apply for the nursing program.

Applicants may apply while completing prerequisite courses but they must all be completed before the start of first semester.

Human Anatomy/Lab or Human Physiology/Lab must be completed with “C” grade or higher before application will be scored. (Fall applicants must have at least one of them completed by end of spring semester, and spring applicants, by end of summer term.)

Applicants must have a current certified nurse assistant certificate, LPN license, or minimum "B" grade in NURS 1005 within 2 years of acceptance into ADN program.  (For example, If NURS 1005 was taken fall 2011  it is good thru Fall 2013 admission) 

Entrance Examination
 

Students will be required to take an admissions exam. This exam will evaluate the student’s projected academic success in the program.  The exam is the Kaplan Nursing School Entrance Exam.  A study guide is available on the Kaplan website, and may be available in the DSC library.   

Kaplan has provided this flyer that gives more information about the test. 

  • Students will be allowed to take the test once per application period.
  • The cost of the test is $20 and must be paid to the Testing Center.
  • The test must be taken within the following dates:   
    • Fall admission:        February 1st - March 30th
      • WALK-INS UNTIL MARCH 9TH ONLY– Tuesday 9:00 am - 2:00 pm, Wednesday through Friday 9:00 am - 9:00 pm.  (Must arrive before 6:00 pm to allow full time for the exam
      •  MARCH 19TH THRU 30TH – BY APPOINTMENT ONLY –  Contact Linda at the Testing Center, 435.652.7696 to reserve a seat on a specific date and time. Please plan ahead as seats will fill up quickly. 
      • Be aware that the testing center may be closed during spring break.
    • Spring admission:   July 15th - September 14th
      • WALK-INS  UNTIL AUG 31ST ONLY– Tuesday 9:00 am - 2:00 pm, Wednesday through Friday 9:00 am - 9:00 pm.  (Must arrive before 6:00 pm to allow full time for the exam
      •  SEPT 1ST THRU SEPT 14TH – BY APPOINTMENT ONLY –  Contact Linda at the Testing Center, 435.652.7696 to reserve a seat on a specific date and time. Please plan ahead as seats will fill up quickly. 
  • Out of area students may arrange for a proctored test locally. All proctors must be set up thru Linda at the DSC Testing Center.  435.652.7696 or email testcenter@dixie.edu 
  • Allow at least 3 days for processing.  
  • NO OUT OF AREA TESTS WILL BE ARRANGED AFTER SEPT 10TH (spring admission) or MAR 27TH (fall admission)
  • The test will consist of 91 questions in four areas as outlined below. There is a calculator within the test that you may use on the math section.
    • Math skills                            28 questions        45 minutes
    • Science                                20 questions        30 minutes
    • Reading comprehension         22 questions        45 minutes
    • English writing                       21 questions        45 minutes

Program Essay
Because the nursing program is writing intensive, applicants are required to write an essay to demonstrate their writing abilities. More information is included in the application packet.

  • The essay is to be completed at the Testing Center at Dixie State College of Utah.
    • Out of area students will be required to find a suitable proctor for the essay. Prior approval must be received from the nursing department. Have the proctor email Alice Clegg, Nursing Advisor, directly for instructions and permission. Please allow at least 3 days notice.
    • NO OUT OF AREA PROCTORS WILL BE ARRANGED AFTER SEPT 10TH (spring admission) or MAR 27TH (fall admission)
  • The maximum length is not to exceed 2 pages, double spaced, word processed.
  • Spelling and grammar check may be used, but no other outside resources.  
  • Essays must be written within these dates:
    • Fall Admission:           February 1st - March 30th 
    • Spring Admission:      July 15th - September 14th
  • There is a 2 hour time limit for the essay.
  • Essays will be evaluated on your ability to: (see scoring rubric)
    • Follow instructions
    • Collect and organize your thoughts
    • Express yourself through writing
    • Show understanding of the role of a nurse
    • Create a logical, concise essay, demonstrating command of the English language
    • Utilize proper grammar and spelling
  • Discuss the following in your essay:
    1. Briefly tell us about yourself and include your professional goals, both long and short term.
    2. Describe the characteristics of an excellent nurse.   Discuss why those characteristics are important to providing safe and competent nursing care..   

Health Requirements
After acceptance into the program, students will be required to meet and maintain certain health and safety standards. Specific details regarding these requirements will be given to students after acceptance. These include:

  • Healthcare provider level CPR certification (American Red Cross or American Heart Association)
    • Classes available thru DSC  435.879.4951 or DXATC  435.652.7730
  • 5 Panel drug screen - Cost: $30-$50
    • DAT express, OHCI or IHC Workmed do these locally for $20-30.  Allow 2 weeks for results to be sent to nursing office.
  • Immunization for:
    • MMR (Measles, Mumps & Rubella)
      • Verification of 2 immunizations, or positive antibody titer.
    • HEP B - Series of 3 required - First must be completed at time of acceptance and second before start of program.  Must complete all three by first semester - (Allow 1 month between first and second, and 6 months between first and third)
    • TDaP  (Tetanus, Diptheria, & Pertussis)
      • Must include Pertussis and after age 18.  
      • Childhood DPT immunization or Adult TD are NOT acceptable
    • Chicken Pox
      • Verification of 2 immunizations 28 days apart
      • or record of disease
      • or signed statement that you had Chicken Pox  (self, parent or doctor)
    • Negative Baseline 2-step TB test  (once upon admission) 2-step includes 2 separate placements and a final negative reading. 
      • May meet requirement with negative chest x-ray within past 5 years, or record of prior treatment for TB.
    • Influenza (seasonal)   Must be done annually during flu season and before November 1st

Complete and submit an application for admission and enrollment to the admissions office. Students are required to submit previous college or university transcripts, high school transcripts, or GED test scores.  All applicants must have applied to DSC before being able to apply for the nursing programs.

The BSN program is designed to be completed in 21 months and starts each fall semester. Students may use spring and/or summer semesters prior to admission for completion of general education and support courses not yet completed.

Applications will be accepted April 1st - June 1st.

Applicants will be admitted into the BSN program without discrimination as to age, sex, creed, ethnic origin, or marital status and must be able to demonstrate physical abilities sufficient to pass each nursing course. Admission to Dixie State College of Utah is also required.

Due to limited positions, some eligible applicants may not be accepted into the program. They may reapply to be considered for future admission.

Academic Excellence

Applicant may be evaluated on overall GPA, nursing course GPA and support course GPA. Nursing courses and support courses must be completed with at least a "C" grade (2.0 GPA), and with a cumulative GPA of at least 3.0

Current RN Licensure
Applicants must provide a copy of current RN license, or a letter documenting the approximate planned NCLEX-RN® testing date.

New graduates may register for first semester classes but must obtain licensure before registering for classes the second semester.

    3. What strategies you will use to fulfill the intensive requirements of the BSN Program
    4. How you feel the BSN Program will assist you in achieving your career goals
Personal References
Applicant must submit three letters of recommendation (forms are included in the application packet) from current or previous nursing supervisor(s), or nursing instructors who have observed the applicant's past performance.
